Medical

• Safety Town Authorization to Administer Medication(s) and Release of Liability Used if medication (liquid, tablet, inhaler, etc.) must be taken during the Safety Town program day. Email or bring form Monday morning.

Alternative Pick-Up Form

• Change of Alternative Pick-ups Add or remove an authorized "Alternate Person." Form can be brought to Safety Town Monday morning or dropped off any subsequent days.

Scholarships

• Email scholarship@sbsafetytown.org PRIOR to registering online. You will be notified if a scholarship is awarded to your child.
